You may upload:

  • Negative PCR COVID Test if taken outside of ECRCHS (read below for more information)

  • Proof of Vaccination (optional) (read below for more information)

COVID-19 Vaccination

Do Students & Staff need to upload COVID Vaccination Records?

  • At this time, uploading proof of vaccination is optional. All students and staff are required to participate in weekly COVID testing regardless of vaccination status.

  • When uploading proof of vaccination, we must be able to clearly see all vaccination information, including: Name, Date of Birth, dose #1 date & manufacturer, dose #2 date & manufacturer (if applicable), and dose #3 date & manufacturer (if applicable). There are two options for uploading your vaccination record:

      • OPTION 1 (recommended): Submit a download or screenshot of your SMART Health Card QR Code and COVID Vaccination information. Several vaccine providers have made this option available. If you were vaccinated anywhere in California, you can download this from the California Department of Public Health Digital COVID-19 Vaccine Record website: https://myvaccinerecord.cdph.ca.gov/

          • Please see the sample screenshot below.

      • OPTION 2: Take a clear photo in good lighting of your 4x3 paper CDC COVID-19 Vaccination Record. We must be able to read all information on the card.

          • If additional doses are on a separate card, take a single photo with all cards visible.

Providing a SMART Health Card QR Code and COVID Vaccination information will allow for faster processing.

Students must use their MyECR Email and Password in order to access the Upload Documents page. This page can be accessed and completed on a computer, laptop, or mobile device.

STEP 1

Use your phone or a computer and go to royalpass.ecrchs.net.

STEP 2

Click on "Upload Documents."

STEP 3

Click on the "Sign in with Google" button.

STEP 4

If prompted, select the Google Account with your ECRCHS email address. If this is not an option, select "Use another account" and sign in with your ECR email.

STEP 5

Complete one of the online forms:

  • PCR COVID Test:

      • If you took your COVID Test outside of ECRCHS, click the green "Upload COVID Test" button to upload your results here

  • COVID-19 Vaccination:

      • Upload proof of your COVID-19 Vaccination (see options for uploading above)

      • Add a mobile phone number

STEP 6

When finished, click "Save" at the bottom of the form.
